
Chief Complaint
A 16 year old female was referred to NYCOMS for the evaluation and treatment of a skeletal open bite. This condition had caused difficulty in incising and masticating a normal diet. It had caused severe pain and spasm in the muscles of the jaw and had affected the patients speech.
Regional Examination
A clinical examination revealed a patient with a considerably large open bite, this had caused the lower jaw to rotate open. The soft tissues of the face were strained, indeed her lips could not close without considerable muscular activity.
Radiographic Evaluation
Cephalometric evaluation demonstrated a skeletal open bite with clockwise rotation of the mandible.

Treatment Plan
After consultation with the patient’s orthodontist the following treatment plan evolved:
- Extraction of maxillary first bicuspid teeth
- Orthodontics to align the maxilla in segments.
- Surgical Phase Le Fort I
a.)Segmental Le Fort I Osteotomy b.)Vertical Reduction Advancement Genioplasty

Outcome
Not only did the patient undergo successful surgery but she was kind enough to document her post-operative course photographically.
